This item will be applied on use if the [[Captain_Steele|player character]] has testicles or a penis. Hitting or exceeding the normal cap will also cause the player to gain a new pair of testicles.

It is interesting to note that the formula always uses the ''modified'' [[Testicle#Size|Testicle Size]] in all of its calculations, and only substract the ball size modifier when calculating the '''Final Testicle Size'''. The ball size modifier is primarily used to keep track of the effects of [[Perks_by_source#Nuki_Nuts|'Nuki Nuts]], so that perk could be used to "simulate" extreme testicle sizes when using that item (regardless of the size they should have), except the transformation only occurs immeditately following a masturbation scene, which would see the modifier be reset to 0.
