It seems that in order to use it, one simply needs to hold it near exposed nipples and press the button. Usage

This item will initiate masturbation on use and will attempt to draw milk from nipples on the player character's breasts.
If enough milk is produced, the milk will be pumped into a milk bag that can then be sold for credits or held in the inventory.
If too much milk is pumped, the milk bag breaks and is lost.
Using this item can also have a random chance of increasing nipple length or areola width.
